Every one of the 27 amendments has been proposed by Congress; the convention route has never been used, and how such a convention would be limited is unsettled. Ratification is by state legislatures or by state conventions, as Congress directs. The deliberately high thresholds are why so much constitutional change happens through interpretation instead.
